Are you in a probationary period? ;look at your contract of employment.
You don't have to use your previous employer for a reference .
If you have already informed your employer that you aren't happy, then it won't come as any surprise to them. The fact that you aren't happy with your living environment is a genuine reason alongside the fact that you are suffering with anxiety. All contributing to how you feel really
It is just the situation that you have found yourself in and so shouldn't really be held against you by your employer.
They could even let you go due to the reasons of your sick note.

Hi All,
I have been in my jobs for maybe 4 months - I have previously posted as to how it isn't really a job and I sit there all day with nothing to do.
I explained to them I am anxious and have been signed off for nearly a month. I am bored and want to return to work but I don't feel returning to that job will help me at all.
I have had a few interviews etc and hoping to have a job offer this week.
If I quit do you think I will have to work my notice? - My GP has been quite understanding that the job itself is the current cause/ trigger for my anxiety.
I was doing quite well and recovering steadily up until taking on this role.
Any advice on how I can quit what to say to my boss.
I don't want to have been on sick leave for a month quit and him quickly receive a reference request.
I have explained to him the job itself is a source of anxiety and I was considering quitting about 2 weeks ago.
I have also said that I dislike living and working in the town I currently live and want to move (due to crazy Ex's having followed me and moved in just a few minutes away)
Any help/advise/support kind words appreciated.
