Nutcracker syndrome is a compression of the left renal vein between the aorta and the superior mesenteric artery. In rare cases (involving the retro-aortic renal vein), it is a compression between the aorta and the vertebrae. Blood forces its way through the left gonadal vein, which gradually dilates, causing blood stagnation in the pelvis. This leads to pelvic varices in women and varicoceles in men.
